Respected Sir/Ma'am,

I am Akshee Batra, an ECFMG certified alien physician selected by XYZ  Medical Center, State, US. I’ll be working as an essential frontline health care provider in the United States. I’m travelling at the invitation of the United States Government for a purpose related to containment or mitigation of virus as outlined in the attached letter by ECFMG. My details are as follows-

·       Last name: Batra

·       First name: Akshee

·       Date of birth: MM/DD/YYYY

·       Place of birth: Delhi, India

·       Country of citizenship: India

·       Passport number: ZXXXXX

·       Visa Number* and Category: PXXXXX and J1

·       Travel dates: Month DD, 2021- India to USA

·       Travel purpose and national interest category, including a clear justification for receipt of a NIE: Alien Physician traveling at the invitation of the United States Government for a purpose related to containment or mitigation of the virus as exempted in Sec. 2 vii of the proclamation (Letter attached).

 

Extra Precautions- I am fully vaccinated. I have taken 2 doses of vaccination with COVAXIN (attached final certificate). I’ll also be in quarantine for 14 days irrespective of COVID test or vaccination before my SSN appointment on Month DD, 2021 (attached program itinerary sent by program coordinator).
